REQUIREMENTS for Travel Document

1. Duly-accomplished Application Form;
2. Four (4) recent passport-size photos (3.5cm x 4.5cm) colored with white background;
3. Copy of data page of Philippine passport. For lost passports, copy of Philippine passport, if available, with duly filled-out Affidavit of Explanation on the loss of Passport or other documents showing Philippine citizenship;
4. Copy of travel itinerary or airline tickets (no layovers, no connecting flights from Canada to Philippines);
5. Copy of immigration status in Canada (e.g. PR card, work permit, study permit, tourist visa or visitor record);
6. If applicable, copy of deportation/ removal order;
7. Proof of urgency of travel (ex., Death Certificate, Medical Certificate, etc. of relatives in the Philippines).

Payment of CAD 46.50 fee (effective  as of 26 February 2025), in cash or bank draft/postal money order payable to the Philippine Consulate General.

Processing at the Consulate takes one (1) business day upon receipt of the complete documents.
