REQUIREMENTS for Renewing a Philippine Passport (18 years old and above)
1. Confirmed online appointment. You may book your appointment through appointment.vancouverpcg.org.
2. Duly accomplished Application Form No. 2 You may download the file here.
3. Personal appearance. The applicant must come in proper business attire with a collar and sleeves. The applicant will be required to take off their eyeglasses, contact lenses and/or facial piercings and must be without heavy make-up, eyeliner and false eyelashes during photo, biometrics and data capture. Facial expression should be neutral with the mouth closed. Ears should also be visible. Forehead must be clear. Bangs/hair fringes must not cover the forehead or touch the eyebrows.).
4. Latest Philippine passport with photocopy of the data page showing the applicant’s photo and complete name and details. If your latest passport is a brown, green, or machine-readable passport, please go to the "Special Cases" tab above.
5. Original and photocopy of at least one (1) of the following proofs of Philippine citizenship, whichever is applicable:
- Dual Citizenship Oath of Allegiance, Identification Certificate or Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI) for those who reacquired or retained their Philippine citizenship as specified in RA 9225;
- For an applicant who possesses dual citizenship from birth (i.e. one parent is a Filipino while the other parent is a citizen from which the applicant’s other citizenship is derived; or have both Filipino parents but was born in a country which applies jus soli citizenship; etc.), a PSA-authenticated Birth Certificate/ PSA-authenticated Report of Birth is required;
- Applicant’s valid Permanent Resident (PR) card;
- Status Verification from IRCC if PR card has expired for more than two years from date of passport application;
- Valid confirmation of Permanent Residency for newly-landed immigrant;
- Latest valid Canadian Tourist Visa or valid Work Permit or valid Student Permit
Please note that the items above are the minimum set of requirements for a passport application. The Consulate reserves the right to request additional requirements for certain situations. To check if you fall under any of these special cases, please click the 'Special Cases' section of this page.
ADDITIONAL REQUIREMENTS FOR SPECIAL CASES
If you fall under any of the special cases listed below, you will need to comply with the additional requirements that are relevant for your specific case:
Special Cases - Document-related
- Applicants who have a brown, green, or machine-readable passport;
- Applicants who are applying for the replacement of a lost, stolen, unretrievable, or damaged passport, regardless of validity / expiration status;
- Applicants who have no PSA-issued Certificate of Live Birth or Report of Birth
- Applicants whose PSA-issued documents are unreadable
- Applicants whose PSA-issued Birth Certificate or Report of Birth is lacking information
- Applicants who have discrepancies in data in the Birth Certificate / Report of Birth and other documents
- Applicants whose birth certificate was registered less than ten (10) years ago from date of application
Special Cases - Changes in surname related to marriage
- Female applicants who wish to use their husband’s surname
- Female applicants who wish to change the configuration of their married name
- Married female applicants who wish to revert to the use of her maiden name due to the death of the spouse
- Married female applicants who wish to revert to the use of her maiden name due to declaration of nullity of marriage, judicially-recognized foreign divorce
- Married female applicants who wish to revert to the use of her maiden name for other reasons
The Consular Officer reserves the right to require additional documents based on the specific circumstances of the application.
Applicant has no PSA-issued Certificate of Live Birth or Report of Birth
1. Applicant must first file for late registration with Local Civil Registrar (LCR) or Consular Office with jurisdiction over the place where applicant was born; and
2. Applicant must then submit PSA-issued late registered Certificate of Live Birth or Report of Birth once available.

Applicant’s PSA-issued documents are unreadable
Applicant must submit any of the following that reflects the unreadable entry/entries, as applicable:
1. Unreadable PSA-issued Certificate of Live Birth (COLB) or Certificate of Foundling: Any of the following:
- Local Civil Registry Municipal Form No. 102
- Civil Registry Form 1-A;
- If born abroad, a copy of the Report of Birth is required.
2. Unreadable PSA-issued Certificate of Marriage:
- Local Civil Registry Municipal Form No. 97
- If married abroad, a copy of the Report of Marriage is required.
3. Unreadable PSA-issued Certificate of Death (in case of widowed applicants):
- Local Civil Registry Municipal Form No. 103

Applicants who lack data in the Birth Certificate or Report of Birth
Applicant must submit any of the following that reflects the lacking entry/entries, as applicable:
- PSA-annotated Certificate of Live Birth;
- If born abroad: PSA-annotated Report of Birth;
- PSA-annotated Certificate of Foundling;

Applicants who have discrepancies in data in the Birth Certificate / Report of Birth and other documents
Applicant must submit any of the following as applicable:
1. Discrepancy is in the applicant’s first name in the Birth Certificate: The applicant must submit any of the following reflecting the correct entry:
- PSA-annotated Certificate of Live Birth
- PSA-annotated Report of Birth
- PSA-annotated Certificate of Foundling.
2. Discrepancy is in data other than the first name of the applicant in the Birth Certificate. The applicant must submit the following:
- Annotated birth certificate authenticated by the PSA reflecting the corrected entry.
Note: If the discrepant data is in the applicant’s other documents, his/her PSA issued birth certificate shall be followed.

Applicants whose birth certificate was registered less than ten (10) years ago from date of application
Applicant must submit any of the following as applicable:
1. Additional valid Philippine Government-issued ID
2. If a valid Philippine Government-issued ID is not available, the applicant must submit two (2) of the following documents:
- NBI Clearance (valid or expired)
- School records such as Form 137-A, Transcript of Records, Diploma obtained from Elementary, High School and/or college
- Service Record if applicant is a Philippine government employee
- Member Data Record from PhilHealth

Applicants who have a brown, green, or machine-readable passport
Applicants must submit all of the following in addition to the basic requirements listed above:
1. Original and photocopy of PSA-issued Birth Certificate OR PSA-issued Report of Birth;
2. Original and photocopy of PSA-issued Marriage Certificate/ PSA-issued Report of Marriage (for married female applicants);
3. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI);
4. Original and photocopy of one (1) valid, government-issued ID.

Applicants who are applying for the replacement of a lost, stolen, unretrievable, or damaged passport, regardless of validity / expiration status
Applicants must submit all of the following in addition to the basic requirements listed above:
1. Police Report in English;
2. Notarized affidavits in English for the specific circumstance. These affidavits may be notarized by the Consulate for a separate fee:
3. For lost, stolen, damaged, or misplaced passports: Notarized Affidavit of Explanation for Loss / Destruction of Passport.
4. Original and photocopy of PSA-issued birth certificate. For married female applicants, original and photocopy of PSA-issued marriage certificate must also be submitted.
5. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI).
6. Photocopy of latest/lost passport, if available.
7. Original and photocopy of one (1) valid, government-issued ID.

Female applicants who wish to use their husband’s surname
Applicants must submit all of the following in addition to the basic requirements listed above:
1. Original and photocopies of PSA-issued Birth Certificate/Report of Birth;
2. Original and photocopies of any of the following:
- PSA-issued Marriage Certificate (if married in the Philippines)
- PSA-issued Report of Marriage (if married abroad)
3. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI).
4. Original and photocopy of one (1) valid, government-issued ID.

Female applicants who wish to change the configuration of their married name
1. Original and photocopies of PSA-issued Birth Certificate/Report of Birth;
2. Original and photocopies of any of the following:
- PSA-issued Marriage Certificate (if married in the Philippines)
- PSA-issued Report of Marriage (if married abroad)
3. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI).
4. Original and photocopy of one (1) valid, government-issued ID.
5. Notarized Affidavit explaining the change in the configuration of the married name.

Married female applicants who wish to revert to the use of her maiden name due to the death of the spouse
Applicants must submit all of the following in addition to the basic requirements listed above:
1. Original and photocopies of PSA-issued Birth Certificate/Report of Birth;
2. Original and photocopies of any of the following:
- PSA-issued Certificate of Death
- PSA-issued or Report of Death (ROD) of spouse, or;
- Apostillized or Authenticated Foreign Death Certificate of foreign spouse with English translation, as applicable, and;
3. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI).
4. Notarized Waiver Form for Reversion to Maiden Name;
5. Latest issued Philippine passport (if available).
6. Original and photocopy of one (1) valid government-issued ID.

Married female applicants who wish to revert to the use of her maiden name due to declaration of nullity of marriage, judicially-recognized foreign divorce
Applicants must submit all of the following in addition to the basic requirements listed above:
1. Original and photocopies of PSA-issued Birth Certificate/Report of Birth;
2. Original and photocopies of any of the following containing a PSA-annotation reflecting the nullity or dissolution of marriage:
- PSA-issued Marriage Certificate (if married in the Philippines),or;
- PSA-issued Report of Marriage (if married abroad);
3. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI).
4. Notarized Waiver Form for Reversion to Maiden Name;
5. Latest issued Philippine passport (if available)
6. Original and photocopy of one (1) valid, government-issued ID.

Married female applicants who wish to revert to the use of her maiden name for other reasons
Applicants must submit all of the following in addition to the basic requirements listed above. Note that this mode of reversion may only be done once:
1. Original and photocopies of PSA-issued Birth Certificate/Report of Birth;
2. Notarized Affidavit of Explanation that includes request for the reversion of maiden name in the Philippine passport or travel document and stating she has not hitherto availed of the reversion;
3. For dual citizens: any of the following documents must also be provided:
- Original and photocopy of the Dual Citizenship Oath of Allegiance;
- Original and photocopy of Identification Certificate, or;
- Original and photocopy of Order of Approval issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI).
4. Latest issued Philippine passport (if available);
5. Notarized Waiver Form for Reversion to Maiden Name;
6. Any existing government-issued valid ID accepted for passport application reflecting the maiden name.

PASSPORT FEES
- Regular Passport Application Fee: CAN$ 87.00
2. Replacement Fee for Lost , Unretrievable, or Damaged Philippine Passports:
- Lost New Generation ePassport: CAN$ 217.50
- Lost brown, green, or machine-readable Passport: CAN$ 130.50
3. Notarization by the Consulate of Affidavit of Loss/Destruction of Passport/ Explanation: CAN$ 36.25
MODES OF PAYMENT
Applicants may pay the fees only through the following methods:
- Cash
- Money Order or Bank Draft payable to the Philippine Consulate General.
Personal cheques, debit or credit cards are currently not accepted
Applicants who apply for a passport during a consular outreach may pay through Money Order or Bank Draft payable to the Philippine Consulate General.
RELEASE OF PASSPORTS
- The new passport shall be available in approximately 8 to 10 weeks after the date of application.
- A self-addressed prepaid return envelope (with tracking number) PER APPLICANT must be provided upon submission of passport application. The applicant may secure said prepaid envelope from Canada Post (Xpresspost envelope) or from a courier service of their choice.
- The Consulate does not assume responsibility for passports/documents that may be lost or damaged in the mail.
