REQUIREMENTS for NEW Passport Application (Below 18 years old)

1. Confirmed online appointment. You may book your appointment through appointment.vancouverpcg.org.

2. Duly accomplished Application Form No. 3. You may download the file here.

3. Personal appearance by the minor and either parent. The applicant must come in proper business attire (with collar and sleeves. The applicant will be required to take off their eyeglasses, contact lenses and/or facial piercings and must be without heavy make-up, eyeliner and false eyelashes during photo, biometrics and data capture. Facial expression should be neutral with the mouth closed. Ears should also be visible. Forehead must be clear. Bangs/hair fringes must not cover the forehead or touch the eyebrows. In the event neither parent can accompany the minor applicant, please go to the "Special Cases" tab.

4. Original and photocopy of any of the following documents. Applicants may order these documents through https://psa.gov.ph:

  • Philippine Statistics Authority (PSA)-issued Birth Certificate
  • Philippine Statistics Authority (PSA) Authenticated Report of Birth.

5. Original and photocopy of latest passport and/or valid government-issued photo ID of both parents. If the accompanying parent is an alien, the non-Filipino parent must present his or her foreign passport and a copy of the Filipino parent’s Philippine passport or other competent proof of identity.

6. Original and photocopy of at least one (1) of the following proofs of Philippine citizenship, whichever is applicable:

  • Dual Citizenship Oath of Allegiance, Identification Certificate or Order of Approval of the dual citizen parent/s issued by the Philippine Foreign Service Post (i.e., Philippine Embassy or Consulate General) or the Bureau of Immigration (BI) for those who reacquired or retained their Philippine citizenship as specified in RA 9225;
  • For an applicant who possesses dual citizenship from birth (i.e. one parent is a Filipino while the other parent is a citizen from which the applicant’s other citizenship is derived; or have both Filipino parents but was born in a country which applies jus soli citizenship; etc.), a PSA-authenticated Birth Certificate/ PSA-authenticated Report of Birth is required;
